Make sure all fields are filled in with accurate information about you and your ban.  Failure to properly fill out the form will result in declination.  Your appeal will be reviewed by Bonz™ or †Blade†.  These are the only 2 people who will review a ban.  They will investigate with the person responsible for your banning.  After reviewing you will get a reply, which usally means within 24 hours.  Do not repeat appeal if yours is declined.  Repeat appeals will be dealt with accordingly.
